Dear Parents & Guardians,
How exciting for your student to be going to Washington D. C. for the 8th grade trip!!
I will be coordinating the medications and medical needs of the students in advance of the trip. There is quite a bit of paperwork for me to review and process. Please turn it in as soon as possible.
The Deadline to Submit Medication Paperwork is:

Medication Rules for D. C.
- Authorization paperwork signed by the doctor[1] is required (ORC 3313.713).
- Authorization paperwork requires a co-signature by the parent.
- Necessary forms are linked below for your convenience
- The nurse needs to review and approve all paperwork.
- Staff are required to keep and administer ALL PRESCRIPTION medicines.
- Exception: Special laws allow doctors to authorize student possession & use of inhalers and epi-auto injectors.
- The original pharmacy labeled container is required for all medication[2].
- Please DO NOT send extra doses of medicine.
- If your student already has medication paperwork on file for the 2021-2022 school year, you DO NOT need to re-submit those forms or the medicine currently in the clinic
- We will attempt to return empty bottles and unused medicines to students at parent pick-up. Otherwise, please pick them up in the school clinic by May 9th. They will be destroyed after that.
[1] The only exception is over the counter medication.
